<h2 class="wp-block-heading">Spontaneous Intracranial Hypotension</h2>



<p class="has-medium-font-size wp-block-paragraph">Spontaneous Intracranial Hypotension (SIH) is a condition characterized by a decrease in the pressure of the cerebrospinal fluid (CSF) that surrounds the brain and spinal cord, occurring without any significant trauma or medical intervention. This low pressure typically results from fluid leaking through a spontaneous small tear or hole in the membrane surrounding the spinal canal (the dura mater). The reduction in CSF volume causes the brain to lose its "cushioning" support within the skull, leading the brain to sag downward and exert pressure on pain-sensitive structures.</p>



<p class="has-medium-font-size wp-block-paragraph">The most typical and diagnostic symptom of the condition is an "orthostatic headache." This pain begins severely shortly after a person stands up or sits down but disappears completely or significantly subsides when lying down. The headache is frequently accompanied by symptoms such as neck pain, nausea, sensitivity to light and sound, ringing in the ears (tinnitus), or hearing changes. Contrast-enhanced brain and spinal MRI imaging are primarily used in the diagnostic process; characteristic signs such as enhancement of the meninges and brain displacement are sought in the MRI findings.</p>



<p class="has-medium-font-size wp-block-paragraph">The first step in the treatment of SIH usually involves conservative (non-surgical) methods such as bed rest, high fluid intake, and caffeine consumption. In cases where these methods are insufficient or the leak persists, a procedure called an "Epidural Blood Patch" is performed. In this procedure, the patient's own blood is injected into the leakage site in the spinal canal to act as a plug, allowing the tear to close on its own. In rarer and more persistent cases, microsurgical methods are utilized to directly close the site of the leak. Early diagnosis is critical for rapidly restoring the patient's quality of life.</p>

<h2 class="wp-block-heading"><strong>Basic Anatomy of Central Nervous System</strong></h2>



<p class="wp-block-paragraph">The nervous system is generally divided into the central nervous system and the peripheral nervous system. The central nervous system consists of the brain, brainstem, cerebellum, and spinal cord. The peripheral nervous system consists of spinal nerves originating from the spinal cord and the autonomic nervous system.</p>



<h2 class="wp-block-heading">MENINGES (BRAIN MEMBRANES)</h2>



<p class="wp-block-paragraph">The brain and spinal cord are located within a bone structure called the cranium and spinal canal, which protects it from impacts. They are enveloped by the meninges, which are three layers of membranes. From outer to inner layers: dura mater, arachnoid mater, and pia mater. The dura mater is a thick, whitish in color, and non-elastic structure. Beneath it lies the arachnoid mater, a thinner, more transparent layer. Below the arachnoid mater is the pia mater, an extremely thin membrane that surrounds all the folds of the brain and spinal cord. Between the dura and arachnoid layers lies the subdural space, and between the arachnoid and pia layers lies the subarachnoid space.</p>

<h2 class="wp-block-heading"><strong>BRAIN</strong></h2>



<p class="wp-block-paragraph">The brain is divided into two main parts: the right and left hemispheres. These hemispheres are connected to each other by special structures in specific locations. The internal structure of the brain consists of gray and white matter. The gray matter forms the outer layer, called the cortex, which is composed of numerous nerve cells (neurons). Thickness of this layer is approximately 2-5 mm. This structure is highly convoluted, consisting of numerous ridges and grooves. Beneath the cortex lies the white matter, which consists of the connecting fibers among neurons.</p>



<p class="wp-block-paragraph">The right and left hemispheres of the brain are further divided into specific regions (frontal, parietal, temporal, occipital lobes) based on various characteristics. Each region contains specialized centers that perform specific functions and are interconnected with each other (such as speech center, comprehension center, auditory center, visual center, etc.).</p>



<p class="wp-block-paragraph">Inside the brain, there are small chambers called ventricles, which are interconnected by thin channels. The interior of these cavities is filled with cerebrospinal fluid, the majority of which is produced by specialized structures called choroid plexus within these cavities. Cerebrospinal fluid flows through these channels and reaches the subarachnoid space surrounding the brain and spinal cord, where it is absorbed into the bloodstream via the venous system.</p>

<h2 class="wp-block-heading">Brain Tumors</h2>



<p class="wp-block-paragraph">The causes of brain tumors are not fully understood, but risk factors include radiation exposure, genetic disorders, a family history of tumors, diseases that affect the immune system, stress, and exposure to various environmental carcinogens.</p>



<p class="wp-block-paragraph">It is thought that brain tumors occur as a result of damage or improper function of specific genes located in the chromosomes of cells. This damage or dysfunction in genes can occur as a result of various environmental factors, as well as being congenital due to genetic disorders in some individuals. When the mechanisms that control cell division are damaged, cells begin to divide and multiply uncontrollably and eventually form an abnormal mass, that is, a tumor. A rapidly growing tumor requires more oxygen and nutrients than normal tissue. Therefore, the tumor produces substances that stimulate the growth of blood vessels to meet its own needs for nourishment and oxygen.</p>



<p class="wp-block-paragraph">Brain tumors are divided into two main groups: primary (originating from brain tissue) and metastatic (spread to the brain from elsewhere in the body). Primary brain tumors arise either from the brain tissue or the tissues surrounding it. Additionally, while metastatic tumors are considered malignant (cancerous), primary brain tumors can be malignant or benign (non-cancerous).</p>

<p class="wp-block-paragraph">The symptoms of brain tumors vary greatly depending on the location in the brain. Accompanying symptoms are usually headache that is more severe in the morning, nausea, vomiting, epileptic seizures, personality changes, speech disturbances, hearing impairments, balance disturbances, numbness or weakness in parts of the body, vision problems, and consciousness impairment.</p>



<p class="wp-block-paragraph">Various imaging techniques such as computed tomography (CT) and magnetic resonance imaging (MRI) are used for diagnosis.</p>



<p class="wp-block-paragraph">Treatment methods for brain tumors may involve microsurgery, radiosurgery (gamma knife, cyber knife), radiotherapy (standard external radiotherapy, IMRT, 3DCRT, proton beam therapy), and chemotherapy (drug therapy), either alone or in combination, depending on the tumor type (primary or metastatic, benign or malignant).Each treatment method has its own specific risks and side effects. In addition to these treatment methods, there are also emerging techniques still in the research phase, such as immunotherapy and gene therapy.</p>

<h2 class="wp-block-heading">Pituitary Tumor</h2>



<p class="wp-block-paragraph">The pituitary gland is a vital endocrine gland located at the base of the brain in a small bony cavity called the "sella turcica" (Turkish saddle) and is widely known as the body's "conductor of the orchestra." Pituitary tumors are masses that arise from the abnormal and uncontrolled growth of cells within this gland. The vast majority of these tumors are benign (adenomas) and do not tend to be cancerous. However, they are classified into two primary groups based on whether they secrete hormones: "Functional tumors" disrupt the body's hormonal balance by producing excessive hormones, while "non-functional tumors" do not secrete hormones but cause issues by exerting pressure on surrounding tissues as they enlarge.</p>



<p class="wp-block-paragraph">Symptoms vary across a wide spectrum depending on the type and size of the tumor. If the tumor is functional, systemic symptoms such as excessive weight gain, growth disorders (acromegaly), menstrual irregularities, sexual dysfunction, or elevated cortisol levels (Cushing’s syndrome) may occur, depending on the specific hormone being secreted. In the case of large non-secreting masses, the primary complaint is the "mass effect." Specifically, pressure on the optic nerves located just above the pituitary gland can lead to a narrowing of the visual field (tunnel vision), blurred vision, and severe headaches as the most typical symptoms.</p>



<p class="wp-block-paragraph">The diagnostic process is conducted through the multidisciplinary collaboration of endocrinology and neurosurgery specialists, guided by the patient's symptoms. The functional status of the gland is evaluated by measuring hormone levels through blood and urine tests. For imaging, the gold standard is high-resolution brain MRI (Magnetic Resonance Imaging), which allows for the precise determination of the tumor's millimetric size and its relationship with surrounding tissues. For patients with suspected vision loss, visual field tests are mandatory to assess the extent of the damage.</p>



<p class="wp-block-paragraph">The treatment plan is individualized based on the tumor's size and its impact on the patient's quality of life. For certain types of tumors (such as prolactinomas), it may be possible to shrink the tumor and regulate hormone levels using only medication. However, in cases where there is pressure on the optic nerve or when medication is ineffective, surgical intervention is the primary option. Today, these surgeries are typically performed through the nostrils using "endoscopic transsphenoidal surgery," which allows for the removal of the tumor without opening the skull. When necessary, radiotherapy may be added to the treatment plan to ensure long-term recovery.</p>

<h2 class="wp-block-heading">Head Injuries</h2>



<p class="has-medium-font-size wp-block-paragraph">Head traumas are injuries resulting from blows to the head, falls, traffic accidents, or sports injuries, covering a wide spectrum from simple scalp lacerations to life-threatening brain damage. These traumas can manifest as skull fractures, concussions, brain tissue contusions, or intracranial hemorrhages. Regardless of the intensity of the impact, every head trauma carries a potential neurological risk, as the sudden movement of the brain within the skull can damage nerve cells and blood vessels.</p>



<p class="has-medium-font-size wp-block-paragraph">Symptoms following a head injury may appear immediately or may develop insidiously hours or even days later. While mild cases involve headaches, dizziness, and mild nausea; more serious cases exhibit danger signals known as "red flags," such as projectile vomiting, confusion, severe drowsiness, loss of strength in the arms or legs, and seizures. In particular, short-term memory loss experienced after the moment of impact or inequality in pupil size can be precursors of a serious increase in intracranial pressure or hemorrhage requiring emergency medical intervention.</p>



<p class="has-medium-font-size wp-block-paragraph">The diagnostic process for head traumas is generally conducted through neurological examinations and radiological imaging methods; Computed Tomography (CT) is the gold standard for rapidly detecting acute hemorrhages and fractures. The treatment approach is determined by the severity of the trauma; while close observation and rest are sufficient for mild concussions, emergency surgical intervention may be required in cases of serious hemorrhage to reduce intracranial pressure and protect brain tissue. Early intervention and accurate follow-up are the most critical factors in preventing permanent damage that may occur after trauma and in preserving the patient's quality of life.</p>

Under normal conditions, the parts of the cerebellum should be located entirely within the skull; however, they are pushed downward due to insufficient volume or structural deformity of the skull in this region. This condition creates physical pressure on the brainstem and spinal cord while also obstructing the natural flow of cerebrospinal fluid (CSF), leading to various stresses on the central nervous system.</p>



<p class="wp-block-paragraph">Symptoms are usually noticed during childhood or young adulthood and vary significantly from person to person. The most characteristic symptom is neck and headache that intensifies particularly during actions such as coughing, sneezing, straining, or sudden head movements. In addition, neurological complaints such as loss of balance, decreased fine motor skills in your hands, numbness in the arms and legs, difficulty swallowing, visual disturbances, and sleep apnea may be observed. Many cases are diagnosed incidentally during MRI (Magnetic Resonance Imaging) scans performed for other reasons.</p>



<p class="wp-block-paragraph">The treatment process is planned according to the severity of the symptoms and their impact on the patient's quality of life. However, in cases of severe pain, specific neurological deficits, or the risk of nervous system damage, surgical intervention is the primary solution. Through a surgical procedure called "posterior fossa decompression," pressure at the base of the skull is reduced, more space is created for the cerebellum, and the normal flow of cerebrospinal fluid is restored. Early diagnosis is of critical importance in preventing permanent damage to the nervous system.</p>

<h2 class="wp-block-heading">Parkinson's Disease and Deep Brain Stimulation (DBS)</h2>

<p class="has-medium-font-size wp-block-paragraph">Parkinson's disease is a progressive and chronic neurodegenerative disorder that occurs as a result of the loss of dopamine-producing cells responsible for controlling movements in the brain. Although it is usually seen in individuals over the age of 60, it can also manifest at younger ages due to genetic predisposition or environmental factors. The decrease in dopamine levels in the brain leads to a disruption in communication between nerve cells, directly affecting the body's mobility and evolving into a condition that limits the quality of life over time.</p>



<p class="has-medium-font-size wp-block-paragraph">The most typical symptoms of the disease include tremors in the hands and feet while at rest, muscle stiffness (rigidity), a significant slowing of movements (bradykinesia), and balance disorders. In addition to these physical symptoms, motor issues such as a masked facial expression, walking with small steps, and difficulty in writing, as well as non-motor symptoms like sleep disorders and depression, are frequently observed in patients. Early diagnosis is of vital importance for managing symptoms and slowing the progression rate of the disease.</p>



<p class="has-medium-font-size wp-block-paragraph">The primary and fundamental approach in Parkinson's treatment is medication; this therapy aims to supplement the missing dopamine from an external source or enhance its effectiveness. However, as the disease progresses, the duration of the medication's effect may shorten, and sudden movement limitations between doses ("off" periods) or involuntary body movements called "dyskinesia" may occur. At this stage, where drug therapy is no longer sufficient or serious side effects are observed, surgical intervention options are evaluated to improve the patient's standard of living.</p>



<p class="has-medium-font-size wp-block-paragraph">The most current and successful method of Parkinson's surgery is Deep Brain Stimulation (DBS). In this procedure, thin electrodes are placed into the regions of the brain responsible for movement, and these electrodes are connected to a small battery unit implanted under the collarbone. DBS suppresses abnormal signals by sending regular electrical impulses to the brain, thereby providing control over tremors, contractions, and slowness of movement. Thanks to this method, patients are able to reduce their medication doses and perform their daily tasks much more independently.</p>
